Emergency Contact Information

It is critical to keep your child's emergency contact information up to date with names and phone numbers of people who can pick up you child in the event of illness or injury.  A parent or guardian must be reachable by phone at ANY time in the event of an illness or emergency.

It is also important that you update the emergency contact page with any changes in your child's health history / condition.
